2. Complete your pet’s intake form.
To ensure every detail is squared away for your pet’s comfort and safety, please complete the appropriate intake form:
Complete the Domestic Intake Form for travel within the United States
Complete the International Intake Form for travel to or from any other destination outside the U.S.
Please complete the intake form that applies to your pet’s journey. We use the intake form to collect the critical information we need to facilitate drop off/pick up, document your pet’s medical needs, and other important details to ensure the trip goes smoothly.
